Shapes Transitions

At Shapes Transitions, each young person has an individually tailored programme of care and activities. Support is provided by a small team of fully trained support workers.

We support adults aged 18 to 30 with a wide range of disabilities in their own home, our support workers are trained and have lots of experience working with children with complex medical and/or behavioural needs. 

Our team will also support your young person in their transition to adult services, whether that’s within Diverse Abilities or with another provider.

High standards of care

Although Day Opportunity services are not required to be registered with Ofsted or CQC, our services are run as if they were and we aim to meet those standards

How to access

To access the service, a referral is required from a funding authority. 

Packages are usually agreed through assessment from a local authority or Continuing Health Care.

What to expect

Day care is provided at Lily’s Place during term time – our respite home where facilities include fully equipped bathrooms, rooms with hoists and specialist equipment, a soft play area, sensory room, ball pool, and garden. 

During school holidays we are based at Langside – our fully equipped specialist school. 

Each person has an individualised routine for each session encompassing support to meet all their daily needs as well as providing exciting and engaging activities within the setting or in the community. 

Staff can be trained to support any physio routines needed.
