Coping with CHAOS weekend booking form

Weekend sessions are back open to everyone from all local authorities after we were able to secure some extra funding.
Please complete the following form in full to request weekend sessions. All sessions are held at The Families Centre, Bond Avenue, West Moors, BH22 0LL, 10am - 2:30pm.
You can request as many weekend sessions as you wish. After the closing date we will allocate spaces and you will then receive an email confirming which sessions your family have been allocated, followed by an invoice from our accounts department.
You can cancel sessions up until 1 week before the session with no charge. After this time, we will not be able to issue a refund.
Please note: this form must be submitted by the end of Sunday 19th November.
Personal details collected from these forms, will only be used to provide you with the services described on these forms. We promise to make all reasonable efforts to keep your details secure, and will only share them with third parties working on our behalf, as required.
